Urban Diabetes Care & Outcomes Dashboard, Audit Years 2017–2021

The Urban Diabetes Care & Outcomes Dashboard, Audit Years 2017–2021, aims to monitor diabetes healthcare and outcomes among American Indian/Alaska Native patients with diabetes who receive services from the 31 Urban Indian Organizations (UIOs) participating in the Indian Health Service’s annual Diabetes Care and Outcomes Audit (Urban Diabetes Audit). This dashboard is user-friendly and replaces the annual Urban Diabetes Audit reports. It highlights the strengths and disparities in the health of urban AI/AN patients with diabetes. Information in the dashboard can be used to guide activities such as program planning, program evaluation, and grant writing!

The Diabetes Audit does not reflect changes seen in individual patients but in the current health status of urban American Indians and Alaska Natives in all participating Urban Indian Organizations (UIOs). Our work seeks to highlight the opportunities and acknowledge the challenges for urban Indians who currently live with, manage, treat, prevent, and overcome diabetes. This work is made possible through the Special Diabetes Program for Indians (SDPI).

Special Diabetes Program for Indians

In 1997, Congress responded to the growing burden of type 2 diabetes in the American Indian and Alaska Native population by funding SDPI. As a result of SDPI, inexpensive yet highly cost-saving measures have been established for diabetes care and prevention: the establishment of diabetes-focused clinical teams, diabetes patient registries, culturally tailored diabetes education tools, nutrition services for children and youth, and weight management programs for adults. SDPI has not only saved lives but saved millions of Medicaid dollars through prevention and management of diabetes and associated health problems such as hypertension, cardiovascular disease, retinopathy, neuropathy, and end-stage renal disease.
